
Community Support Services
Community Support Services (CSS) offers a range of support and services for adults diagnosed with severe and persistent mental illness (SPMI). This program provides community-based treatment to assist individuals with mental health conditions such as major depression, bipolar disorder, and schizophrenia.
These services are designed to enhance the ability to live independently, develop skills for maintaining employment, build social relationships, and foster a sense of community. To maximize convenience for our consumers, support is available both at home and in the community.
All of our mental health services are tailored to individual needs, featuring unique treatment plans for each consumer. The CSS department includes various programs, each specifically designed to address different aspects of daily living.
Case Management
Case management is a medically necessary, one-on-one program. The program provides personalized, recovery-based services to support adults who experience mental health needs or have been diagnosed with a serious and persistent mental illness. Case management provides services that help individuals achieve recovery and allows them to function independently in the community. Case managers work to build relationships that foster hope, courage and resilience by building on the individual’s strengths.
Psychosocial Programs
Psychosocial rehabilitation assists individuals in developing the social, emotional, and intellectual skills necessary for a fulfilling life with minimal professional support. This approach employs two key strategies for intervention: teaching coping skills to better manage stressful situations and fostering resources that help reduce future stressors.
Peer Support
Peer Support is a patient-centered service aimed at facilitating recovery and helping individuals return to their previous level of functioning. Services can be offered in one-on-one sessions or group settings. Peer support specialists provide these services, focusing on communal support and the development of essential skills and strategies for recovery. These specialists have lived experience and have completed certified training to effectively assist others in their recovery journey.
Medication Services
At ICHD, our Medication Services team is committed to effectively managing your mental health through the prescription, monitoring, and oversight of psychotropic medications. We confidently address a range of psychiatric conditions, including depression, anxiety, bipolar disorder, and schizophrenia.
Our Medication Services are designed for all clients, and we also provide expert medication management for other key programs within ICHD, such as the Assertive Community Treatment (ACT) program and the Medication Assisted Treatment (MAT) program for alcohol and opioid addiction.
Assertive Community Treatment (ACT)
The ACT program is evidence-based and offers a comprehensive range of treatment and support services for individuals diagnosed with serious mental illnesses who require a higher level of care.

The MAT program is individually tailored to meet the specific needs of patients, utilizing medication in conjunction with therapy to treat addiction and substance use disorders.

Individual Placement & Support (IPS)
Individual Placement and Support (IPS) is an evidence-based approach designed to provide integrated supported employment for individuals with mental illness or co-occurring disorders.
The goal of the IPS program is to help individuals who express a desire to work find employment. The program welcomes all adults aged 18 and older, regardless of the severity or progression of their symptoms or the management of their conditions.

Transitional Living Center
The Transitional Living Center aims to help individuals develop healthy habits that they can maintain in independent living. The Transitional program offers a safe and affordable environment where participants have stable housing, learn essential independent living skills, and receive support for medication compliance.

Independent Housing
In 2010, ICHD opened Oak Haven Plaza, a two-level apartment complex. Each unit includes a living area, kitchen, bathroom, and bedroom. Additionally, the complex features a safe room/community room, laundry room, and communal kitchen.
